Step 1
Review the program dates and ensure that your schedule allows you to attend all classes.
Step 2
Complete the following forms available on this web site:
- CFA application
- Release of information & statement of consent
- Release, waiver, and hold harmless agreement
Step 3
Contact Anna Worman at (614) 652-3924 to schedule a time and date, no later than 4 weeks prior to the start of class, to notarize the release, waiver, and hold harmless agreement, get fingerprinted and have your ID photo taken.
All participants must submit to a fingerprint check through the Bureau of Criminal Identification (BCI) and a record check through the Dublin Police Department. You must bring your driver's license with you to this meeting.

Contact Firefighters Chad Reed at (614) 652-3891 or Don Schleich at (614)
652-3995 to schedule a time and date, no later than 4 weeks prior to the start of class, to have your self-contained breathing apparatus (SCBA) fit test completed.
Step 5
Contact James Ross at (614) 652-3935 to schedule a time and date, no later than 4 weeks prior to the start of class, to have your personal protective equipment (PPE) fitted.
To be considered for the 2008 class, applications must be received and the above steps completed by July 1, 2008.
